Outstanding TCM Physician Award Description

The Outstanding TCM Physician Award recognises TCMPs who achieved Senior Consultant TCMP level as per Career and Remuneration Guidelines (CRG) issued by MOH, and contributed significantly towards clinical work that elevates the quality and safety of TCM treatment, including supporting and facilitating research in his/her hospital, or community. The awardee has also successfully promoted the use of safe and effective treatment methods and advocated preventive health and community care, resulting in a higher quality and standard of TCM service delivery.

Awardee will receive $5,000 prize money, trophy, and a certificate.

Supporting Documents for Nomination

Please provide an Executive Summary in 1-2 pages highlighting the nominee’s main achievements, justification for the award, and a summary of the supporting information stated below on an official letterhead and signed by the Head of organisation.

The remaining supporting information should be included after the Executive Summary and indicated as Annexes with their corresponding heading in the same Word document.

Executive Summary	Summary of the nominee’s achievements	To highlight the nominee’s main achievements, justification for the award category, and a summary of the supporting information presented in Annex A-G. This should be submitted on an official letterhead and signed by the Head of organisation.
Annex A	Fulfilment of MOH CRG criteria	To provide supporting documents to show nominated TCMP has achieved Senior Consultant TCMP level as per Career and Remuneration Guidelines (CRG) issued by MOH.
Annex B	Contributions	To provide justification as to how the nominee has contributed significantly to the TCM community as a dedicated TCM physician, elevating the quality of TCM healthcare delivery: Played a major role in the development of TCM in Singapore and made exceptional contributions to clinical work that advances the safety and quality of TCM patient care and/or community care; Successfully promoted the use of safe and effective treatment methods, resulting in high standard of quality TCM healthcare delivery or innovative methods in improving health outcomes and promoting the health and well-being of the community; Demonstrates consistent and outstanding performance in TCM practice and leadership in advancing and developing his/her field of interest in TCM for the last 15 years; Nominee is a role model to younger TCMPs and engages actively in teaching and training to develop his/her field of interest in TCM and creates an environment where continuous learning is encouraged; Supports and facilitates TCM research; Awarded and commended in both TCM and non-TCM areas.
Annex C	Impact of Contribution and Recognition	To provide justification and attach relevant documents justifying the impact and recognition of the nominee’s contribution and recognition in TCM (e.g. awards received, speaker’s invitation, etc.) on the following levels: Institutional National International
Annex D	Achievements	To provide justification that the nominee is a respected TCMP in his/her respective field of interest in TCM and/or has made significant contributions to the development of TCM in Singapore.
Annex E	Track Record	To provide supporting documents that the nominee demonstrates a consistent track record of publishing in high-impact journals or presentations at recognised platforms.
Annex F	Curriculum Vitae (CV) of Nominee	Please provide the CV of the nominee (up to 5 pages).
Annex G	Other supporting documents	e.g. Letter(s) of recommendation, list of publications and journals, project outline, etc.
