Nomination Forms Guidelines
Application
- Follow the instructions listed in the nomination forms.
- Complete all sections in the nomination forms in English/Chinese. Indicate “NA” if the particular criteria are not applicable.
- Use Arial/Calibri font of at least size 11 for all text and 1 inch / 2.5cm wide margin for the submitted documents. Please ensure that the text in images is legible.
- Each clinic, organisation or institution should submit (i) one Word document per nomination and (ii) a combined PDF submission that includes all nominations across the award categories and endorsed by the management-level (e.g. head, owner, chairman, director) of the clinic, organisation or institution or declared to be self-employed.
- Each Word/PDF file should not exceed 2MB.
- Each nomination (including Annexes) should not exceed 20 pages.
- Each nomination form in one Word document should comprise the following:
- Completed nomination form
- An Executive Summary in 1-2 pages
- Annexes, including CV of each nominee and other supporting documents (e.g. letters of recommendation, list of publications and journals, project outline, etc.)
- Please do not embed documents and/or include hyperlinks in the submitted Word documents and PDF file.
Submission
- The clinic, organisation or institution should submit the soft copies of the Word documents and PDF file to National Exemplary TCMPs Awards (NETA) secretariat by 31 October 2025, 2359H.
- Do not submit password protected Word documents or PDF file. If required, the documents could be submitted in a password protected zip folder.
- Late submission or revision to the submitted nomination forms will not be allowed after the closing date. Incomplete applications will not be considered.
